Forms Required by the First Day of School


Required Forms (All Students): 

Off Campus Activities Permission Form

Your child will be participating in a wide variety of off-campus supervised activities. Students leaving the Synapse School campus do so for a variety of reasons, including outdoor physical education, walks through the neighborhood, and park play.  These activities take place in the North Fair Oaks Community and within a one mile radius of Synapse School.  
Please complete the permission form for these activities.

Brainwave Learning Center Permission Form

Please follow this link to our consent form, which is required for your child to participate in BLC research studies. The consent form is followed by a questionnaire, which will take about 5 minutes to complete. If you prefer a paper copy of the forms or have any questions, please do not hesitate to contact Dr. Liz Toomarian, Director of the BLC at Synapse ( or by phone: 650-391-3897).

Synapse School is happy to offer a shuttle service. Here are the 2019-2020 Synapse Shuttle Program Details, including shuttle policies, rider rosters, etc. We currently have openings at the El Camino Park in Palo Alto and Serra Park Tennis Courts in Sunnyvale.

Students should bring a bag lunch as well as a morning/afternoon snack. Alternatively, parents/guardians can order lunches through our school lunch program, School Foodies. Sign up on with the school code: Synapse. Lunches will be available starting August 22nd when classes begin. Check out this flyer for more information.

After School  

After school activities include After Care, which begins August 22nd (no After Care for Level I Aug. 22 & 23) and runs from dismissal until 6:00pm each day, and After School Programs, which begin Sept. 3. 


After Care

The After Care program is available for students from dismissal - 6:00pm Monday-Friday beginning the first day of school for students in Levels II through Middle School. Please note that After Care will not be available for Level I during the first week of school. After Care will be available for all students beginning Monday, August 26th.

The cost of After Care is $15.00 per day for any child in After Care following the complimentary first 30 minutes. Parents should let Synapse know in advance if your child will be attending After Care by filling out the After Care form found in the weekly newsletter. Sign up now for First Week of School After Care (Level II - MS only).

After School Programs

Registration link and class options for Trimester 1 will be shared in coming days. A preview will be available at the SynC Welcome Coffee on the first day of school, August 22nd. Programs start Sept. 3.

Synapse Giving 

Here is an overview of philanthropy at Synapse. We encourage you to become familiar with the various ways you can support the school. We are so grateful for the help of our wonderful families.

Fall registration for Volleyball (girls) and Cross Country (co-ed) is now open and will close on Friday, August 30th. After signing up, you will be required to complete SSIL and Synapse waivers. For more information, contact Athletic Coordinator, Elizabeth Bloom, at

Middle School parents/guardians: Please fill out this sports interest form with your middle school student(s) in preparation for the upcoming school year.

Immunizations Required by August 1, 2019

California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Parents must show their child’s Immunization Record as proof of immunization.

Email immunization records and forms to

o   All new students: Submit the complete immunization record.

o   Any students who have updated immunization records since submitting last Fall.  

o   7th Graders: Submit the complete immunization record including Tdap booster.

o   Kindergarteners: Submit the complete immunization record.

o   Kindergarteners and/or 1st Graders: Submit a Report of Health Examination for School Entry within 18 months prior to or 90 days following entry into 1st grade, or submit a signed parental waiver of examination.

SynC, Synapse’s parent-teacher organization, has two primary goals: 1) to maintain and continue to develop the incredible community of families at Synapse; and 2) to organize the parent community to better support the teachers and overall school.

Synapse Volunteer Opportunities


There are many opportunities to volunteer at Synapse as a parent/guardian, either with one time events, or throughout the school year. Parent/guardian volunteers help support and show appreciation of our teachers, as well as organize different types of events and activities to help parents connect with one another. If you are interested in volunteering next year or have other interests you would like to offer Synapse, please reach out to the SynC Co-Chairs at
