Immunizations

 

Immunizations Required by August 1, 2019

California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry. Parents must show their child’s Immunization Record as proof of immunization.

Email immunization records and forms to healthrecords@synapseschool.org

o   All new students: Submit the complete immunization record.

o   Any students who have updated immunization records since submitting last Fall.  

o   7th Graders: Submit the complete immunization record including Tdap booster.

o   Kindergarteners: Submit the complete immunization record.

o   Kindergarteners and/or 1st Graders: Submit a Report of Health Examination for School Entry within 18 months prior to or 90 days following entry into 1st grade, or submit a signed parental waiver of examination.

Medications and Medical Conditions

A student may have an illness that does not prevent school attendance, but may require medication for relief of symptoms, cure of the illness, or prevention of further disease. It is recommended that medication be given at home whenever possible. For example, medication prescribed three times a day can be given before school, after school and at bedtime.
 
Synapse School will administer over-the-counter medications and prescription medicine to children ONLY under specific circumstances, as follow:
 
Over the counter medication:
  1. The medication is in the original container.
  2. A PARENT/GUARDIAN PERMISSION TO GIVE OVER-THE-COUNTER MEDICATION form has been completed by the parent/guardian.
  3. A written authorization must be provided by the parent or guardian prior to the application of any topical products.
  4. The parent or guardian must bring the medication to Synapse School receptionist. For safety and security reasons, medicine should not to be sent with the child or sibling. Notes from home will NOT be accepted as authorization for dispensing medications.
 
Prescription medication:
  1. All prescribed medication shall be received and stored in the most current
    labeled container from the pharmacy showing: the student’s name, name
    and strength of drug, directions for administering, and physician’s name.
  2. A PARENT/GUARDIAN AUTHORIZATION TO GIVE PRESCRIPTION MEDICINE form has been completed by the parent/guardian.
  3. The parent or guardian must bring the medication to Synapse School receptionist.  For safety and security reasons, medicine should not to be sent with the child or sibling. Notes from home will not be accepted as authorization for dispensing medications.
